Responsible for the development of a systematic written case plan for each consumer. Establishes links between the consumer and the service providers. Conducts a systematic reevaluation of the consumer's human service needs and progress. Completes appropriate and timely documentation as required in the consumer's record.
Full Time Monday-Friday 8am-5pm with flexibility depending on need.
Bachelor's Degree Psychology, Social Work, and Counseling will be considered.
Must have a valid driver's license with an insurable driving record.
